Final Coordination Questionnaire

We are getting close to the finish line and we are so excited for your wedding day! Throughout the planning process we have likely talked through many of these details during meetings, emails, and check ins. This questionnaire is where everything comes together. It allows us to collect your final decisions in one place so our coordination team knows exactly how you want the day to flow. This typically takes about 45 minutes to an hour to complete. We recommend setting aside a little uninterrupted time and working through it together.

A few things to keep in mind before starting. This form does not save progress, so if the page is closed your answers will not be stored. You may submit the questionnaire at any point and we will receive your responses. If you need to come back to it later, simply start again where you left off and make sure your names are included at the beginning so we can match everything correctly.
